Registration
Here is a registration form you can access and fill out. Note: You should download
the form and bring it by one of the Centers so we have a signed original document.
If you send this to us via email as an attachment (parkerkls@yahoo.com), you will
still be required to sign it on your child's first day. You may fax the completed
form to 479-595-0557. The Parent Handbook can be obtained at a Center.
Additional forms are required when your child is enrolled at Caterpillar Clubhouse.
They relate to the Special Nutrition Program in NWA and field trips.
